Supported Accommodation
For Care Experienced Young People In Herefordshire

Offering a range of support and accommodation for ‘looked after’ 16 and 17 year olds and care leavers up to the age of 25, who are transitioning to adulthood. With the aim of supporting them to develop their independent living skills in order to successfully manage their own accommodation.

Overview

Futures Hfd offers placements for up to 21 young people across a number of different provisions in Herefordshire, offering varying levels of support according to each young
persons individual needs.

Futures Hfd Provision

All of the accommodation is completed to a high standard and comes fully furnished and equipped with everything a young person would need.

Security and safeguarding are a priority for Futures Hfd, with CCTV provided across all communal areas with in The Heart of Oak and Berrington Street, and the front doors of all outreach properties, which can be monitored remotely.

The bespoke accommodation and support packages available across Herefordshire include:

Heart Of Oak

A staffed 24/7 provision for up to 8 young people on the outskirts of the city centre.

Berrington Street

A further 4 one bedroom, self contained flats in the city centre staffed weekdays.

Outreach properties

9 outreach properties 1 & 2 bedroom around Hereford city and surrounding area with outreach support.

Staffing & Training

Our Team have 20+ years experience of social work and working with Looked After children. Our Team possesses recognised qualifications ranging from degrees in Social Work and Early Years to vocational qualifications.

Futures Hfd recognises the importance of relevant and up to date staff training in order to provide good quality, safe and effective support.

Placements

Futures Hfd offers placements to young people for as long as they are needed up to the age of 25. Whilst ideally all placements are planned Futures Hfd recognise this is not always possible and there will be occasions where an emergency or same day placement is needed. Where possible this can be facilitated.  

Referrals

Futures Hfd accepts referrals directly from Local Authority Children’s Services Departments. We are happy to discuss the needs of any young person prior to making a referral by contacting us through the website, email or phone.

Futures Hfd was created and developed in 2020 by two qualified, experienced and registered social workers with the aim of providing good quality, safe accommodation and support for those young people transitioning from care to independent living.

Feedback

Futures Hfd values service user feedback and input from both young people and professionals using the service. By employing range of methods Futures Hfd encourages young people and professionals to share their views,
wishes and feelings which is then used to inform and develop effective service delivery.
